The app provides a missing Contact Us form for the Office 365 website. Visitors fill in their contact information to be stored in SharePoint. Admin has access to the list of submissions and can easily modify form by adding or removing fields. App provides the following features:
Standard list of fields with customizable visability and requirement settings:
Works with public SharePoint sites with anonymous access.
Standard SharePoint email alerts for the list of submissions.
App part can be reused on multiple pages of the site with unique settings and a way to destinguish between submissions from different forms.

Please go through the following steps to set up the Cutomer Requests List Permissions:
Site Contents page.
Contact Us app and click it.
Customer Requests icon on the left portion of the page.
On the opened page select ribbon
LIST option and click
Shared With button.
On the opened popup window click on the
Specify the required permission.
Please go through the following steps to add and configure Contact Us App Part:
Open the page where the Contact Us will be placed.
Edit Page button in the Ribbon.
Select a place (a Web Part Zone) and click
Insert -> App Part in the Ribbon.

After the App is added open its settings and customize contact us form fields visibility and request identification prefix/postfix for new requests.
